Free Mental Health Self-Assessments

Four short, educational self-checks for adults from SLS Psychiatry. Use them to organize what you are noticing before a visit — they are not diagnostic tools and they do not store anything.

Available self-assessments

Adult ADHD self-test

10 questions · ~3 minutes

Adults who struggle with focus, organization, procrastination, time blindness, or chronic disorganization at work, school, or home.

Mental health self-check

10 questions · ~3 minutes

Adults reflecting on common depression and anxiety symptoms over the last two weeks — mood, sleep, energy, appetite, concentration, and worry.

Sleep & mood self-check

10 questions · ~3 minutes

Adults whose sleep difficulties may be affecting mood, focus, or daily energy — including racing thoughts at night and unrefreshing sleep.

Postpartum depression & anxiety self-check

10 questions · ~3 minutes

New parents wondering whether what they are feeling is more than the typical postpartum adjustment — covers mood, anxiety, sleep, bonding, and safety.

How to use these

  • Answer based on roughly the last two weeks. Do not overthink any one item — a rough sense is fine.
  • Nothing is stored or shared. Everything runs in your browser for self-reflection only.
  • The results are educational, not diagnostic. Only a licensed clinician can diagnose a mental health condition after a full evaluation.

Not sure where to start?

You do not have to take a self-check first. If you would like to talk with a psychiatrist, you can request an evaluation directly — Southlake in-person or telehealth anywhere in Texas. Visit our Contact page or call (817) 203-3721.
