Information about our practice, policies, and what to expect — from insurance and self-pay to telehealth, refills, and our adult ADHD evaluation process.

Do you treat children or teens?

SLS Psychiatry currently sees adults 18+ only.

Do you offer therapy?

SLS Psychiatry focuses on psychiatric evaluation and medication management. Supportive discussion and psychoeducation may occur within psychiatric visits, but the practice does not offer ongoing standalone therapy.

Do you accept insurance?

SLS Psychiatry accepts UnitedHealthcare, Blue Cross Blue Shield, Cigna, and Aetna. Curative is currently in process.

Do you offer self-pay?

Yes. Self-pay is available. For some out-of-network patients, a superbill may be provided when appropriate.

What conditions do you treat?

Anxiety, depression, mood symptoms, adult ADHD, PTSD, panic, social anxiety, OCD, sleep concerns, adjustment issues, postpartum depression/anxiety, women's mental health concerns.

Do you offer telehealth?

Yes. Telehealth is available for patients physically located in Texas at the time of the visit.

Do you offer in-person visits?

Yes. In-person visits are available at 305 Miron Drive, Southlake, TX 76092.

How long are appointments?

Initial psychiatric evaluations are typically 60–75 minutes. Follow-up appointments are typically 20–50 minutes.

What happens at the first visit?

During the first visit, the provider reviews your psychiatric history, current concerns, goals, medication history, safety considerations, and treatment options to develop a comprehensive plan.

Do you prescribe medications?

SLS Psychiatry provides medication management when clinically appropriate. No specific medication is guaranteed, and medication decisions are made after clinical evaluation.

Do you prescribe controlled substances?

Controlled substances are never guaranteed. Any medication recommendation is based on clinical evaluation, safety, diagnosis, history, and appropriateness.

What is your approach to adult ADHD evaluation and stimulant medication?

Adult ADHD evaluation at SLS Psychiatry is a clinical process, not a same-day stimulant prescription. The first appointment is a 60–75 minute psychiatric evaluation that reviews current symptoms, developmental and school history, work and relationship impact, family history, prior diagnoses or treatments, and conditions that can mimic ADHD (such as anxiety, depression, sleep disorders, or substance-related concerns). When clinically appropriate, eligible patients may complete a provider-directed Creyos digital cognitive assessment — a validated tool used to support, not replace, the clinical evaluation. Treatment recommendations (which may or may not include stimulant or non-stimulant medication) are made only after this evaluation is complete. We will not prescribe controlled substances at a first visit without a full evaluation, and we do not transfer existing controlled-substance prescriptions sight-unseen. If you are looking for an immediate stimulant prescription without an evaluation, SLS Psychiatry is not the right fit — and we would rather tell you that up front than waste your time and intake fee.

Will my existing stimulant or controlled-substance prescription transfer to SLS?

Not automatically. New patients on existing stimulant, benzodiazepine, or other controlled-substance medications are still seen for a full 60–75 minute psychiatric evaluation before any prescribing decision is made. The provider may continue, adjust, taper, or change a regimen based on that evaluation. Please bring a complete medication list, the prescribing provider's name, and recent records when possible so we can make a safe, informed decision at the first visit.

What is the cancellation policy?

There is a $75 cancellation fee.

How do I request a prescription refill?

The fastest way is to ask your pharmacy to fax the refill request to our office at (817) 270-9347. You can also call (817) 203-3721 or email info@slspsychiatry.com with your name, date of birth, medication, dose, and pharmacy. Please send the request before you take your last few doses. How long do refill requests take?

We aim to process routine refill requests within two business days. Requests sent on Friday afternoon, weekends, or holidays are picked up on the next business day.

Do I need an appointment to get a refill?

Routine refills require a recent visit on file with the provider managing that medication — typically every 1–3 months while we are adjusting treatment, and at least every 3 months once stable. Controlled substances (such as stimulants) and any dose changes always require a scheduled appointment.

What happens to my refills if I miss an appointment?

We do not issue ongoing refills when an appointment has been missed and not rescheduled. Booking a follow-up is the quickest fix — once we have seen you, refills can usually go out the same day.

Can I request a doctor's note?

Patients may request a doctor's note after a visit when clinically appropriate.

Do you complete FMLA paperwork?

For FMLA or complex paperwork, patients are generally encouraged to begin with their primary care physician. SLS Psychiatry may help coordinate additional documentation when clinically appropriate.

What should I do in an emergency?

If this is an emergency or life-threatening situation, call 911 or go to the nearest emergency room. If you are experiencing a mental health crisis, call or text 988.

How is my privacy protected?

SLS Psychiatry follows applicable privacy laws. Patient information is protected and only shared as allowed by law or with patient permission.

Can I be seen after work?

Yes. Josh, our PMHNP, offers evening availability daily from 5 PM to 7 PM and Friday afternoons from 1 PM to 7 PM by appointment. Telehealth visits are available across Texas during these hours, which makes after-work appointments practical for most working adults.

Can I use telehealth if I am traveling?

Telehealth visits are only permitted when the patient is physically located in Texas at the time of the appointment. If you will be outside Texas on your scheduled date, please contact the office in advance to reschedule.

What if I am located outside Texas?

SLS Psychiatry is licensed to provide care in Texas only. If you are physically located outside Texas, the practice cannot conduct an evaluation or follow-up visit on that day. We are happy to help you reschedule for a date when you will be back in Texas.

Can I choose to see Dr. K or Josh?

Yes. Patients can request to schedule with Dr. K or with Josh. Both providers offer comprehensive psychiatric evaluation and ongoing medication management. If you are unsure which provider is the best fit, the office can help guide you based on availability and preferences.

What if my insurance is not accepted?

If we are not in-network with your plan, you may still be seen as a self-pay patient. Upon request, we can provide a superbill — a detailed receipt you can submit to your insurance company for potential out-of-network reimbursement. We recommend confirming your out-of-network mental health benefits with your insurer in advance.

How much do cognitive assessments cost?

There is a one-time $200 Cognitive Assessment Access fee for eligible patients who complete provider-directed Creyos assessments. Please contact the office for payment details. This fee may not be covered by insurance and is separate from appointment fees.

Is the $200 assessment fee included in my visit?

No. The $200 Cognitive Assessment Access fee is a separate, optional one-time fee. It does not cover appointment fees, which are billed separately. Please contact the office for payment details.

Does the assessment diagnose ADHD?

No. Creyos assessments do not diagnose ADHD or any condition by themselves, do not replace a psychiatric evaluation, and do not guarantee medication, stimulants, or controlled-substance prescribing. Results are reviewed by your provider as part of your overall psychiatric care.

Does the assessment guarantee ADHD medication or stimulants?

No. Creyos assessments do not guarantee medication, stimulants, or controlled-substance prescribing. All treatment decisions are based on a full clinical evaluation.

Can I take assessments whenever I want?

Assessments are provider-directed and used when clinically appropriate. They are not intended for unlimited self-testing or emergency use.
