Not in-network with your plan? This includes Texas Medicaid, with which we are not currently contracted. You can still be seen as a self-pay patient with published rates and a Good Faith Estimate, and we provide superbills for out-of-network reimbursement when appropriate.

Self-pay rates

Initial psychiatric evaluation: $350 per hour. Initial evaluations are billed at $350 per hour. Visits typically run 60–75 minutes, so most patients should plan for a charge in that range.

Self-pay patients can request a Good Faith Estimate before their first visit under the federal No Surprises Act.

Out-of-network benefits

If we are not in-network with your insurance provider, you may still be able to receive reimbursement for your visits depending on your plan’s out-of-network benefits.

Upon request, we can provide a detailed receipt known as a “superbill.” You can submit this superbill directly to your insurance company for potential partial reimbursement.

We recommend contacting your insurance provider prior to your visit to ask about your out-of-network coverage for outpatient psychiatric services.

Cognitive Assessment Access fee

Eligible patients receiving cognitive or ADHD-related care may have an optional Cognitive Assessment Access fee for provider-directed Creyos digital assessments. This fee is separate from appointment fees and may not be covered by insurance.

Please contact the office for current pricing and payment details. Creyos assessments do not diagnose ADHD or any condition by themselves and do not replace a psychiatric evaluation.

Which insurance plans does SLS Psychiatry accept?

SLS Psychiatry is in-network with Aetna, Blue Cross Blue Shield, Cigna, UnitedHealthcare, Medicare (all plans), and eligible Curative plans for adult psychiatric care — in person at our Southlake, TX office and by telehealth anywhere in Texas. Curative members can see plan-specific details on our Curative insurance page. Self-pay and out-of-network superbills are also available.

Is SLS Psychiatry accepting new patients?

Yes. Both Dr. Rana Kaleemullah, MD (board-certified psychiatrist) and Josh Fantauzzi, MSN, PMHNP-BC are accepting new adult patients, with appointments in person in Southlake and by secure video across Texas. You can verify your insurance online in about two minutes, and our office confirms your benefits within one business day — before you commit to a visit.

Do you accept Aetna?

Yes. SLS Psychiatry is in-network with Aetna for adult psychiatric evaluations and ongoing medication management, both in person in Southlake and by telehealth across Texas. Copays and deductibles vary by plan, so we verify your specific Aetna benefits before your first appointment.

Do you accept Cigna?

Yes. SLS Psychiatry is in-network with Cigna for adult psychiatric evaluations and medication management, in person in Southlake and by telehealth across Texas. We confirm your plan's cost-sharing during benefits verification so there are no billing surprises.

Do you accept Medicare?

Yes. SLS Psychiatry accepts Medicare (all plans) for adult psychiatric services. If you have a Medicare Advantage or supplemental plan, we verify your specific benefits before your first visit so you know your expected out-of-pocket cost in advance.

Do you accept Medicaid?

We are not currently contracted with Texas Medicaid or its managed-care plans. Patients with Medicaid can still be seen as self-pay: our rates are published on this page, and self-pay patients can request a Good Faith Estimate under the federal No Surprises Act before their first visit.

What if SLS Psychiatry is out of network with my insurance?

You may still be reimbursed through your plan's out-of-network benefits. On request we provide a detailed receipt (a superbill) that you submit to your insurance company for potential partial reimbursement. We recommend calling your insurer first to ask about out-of-network coverage for outpatient psychiatric services.

How do I verify my insurance before booking?

Use our secure online verification form — it takes about two minutes. Our office confirms participation and cost-sharing with your carrier and follows up within one business day. Verification of benefits is an estimate, not a guarantee of coverage or payment.

Does insurance cover telehealth psychiatry visits?

Yes. Our in-network plans (Aetna, Blue Cross Blue Shield, Cigna, UnitedHealthcare, Medicare, and Curative) cover telehealth psychiatric visits billed the same way as in-person care. You must be physically located in Texas at the time of each visit — a state licensure requirement.

Cancellation policy

We value your time and the time of our clinicians. There is a $75 fee for appointments canceled without sufficient notice or for missed appointments.
