Adult Psychiatrist Accepting Curative Insurance in Texas

SLS Psychiatry participates with certain Curative health plans for eligible adult members, effective June 1, 2026 — for psychiatric evaluation and medication management in Southlake or by secure telehealth across Texas.

What is Curative?

Curative is a Texas-founded health insurer offering employer-sponsored health plans. Their model rewards preventive engagement: members who complete an annual Baseline Visit may unlock the Curative Pass benefit, which can reduce or eliminate cost-sharing on covered services including psychiatric care.

Curative offers PPO and EPO plan designs. Your specific benefits — including cost-sharing and prior authorization requirements — depend on your employer’s plan selection and your Baseline Visit status.

PPO, EPO, and Curative Pass

  • PPO: In-network providers at lower cost-sharing; out-of-network at higher cost-sharing. More provider choice flexibility.
  • EPO: In-network only — no out-of-network coverage except emergencies. SLS Psychiatry is in-network.
  • Curative Pass: A benefit feature (not a separate plan) that may activate after completing your annual Baseline Visit, potentially reducing or eliminating cost-sharing on covered services.

The Baseline Visit requirement

Curative’s Baseline Visit is an annual wellness exam — typically with your primary care provider — that unlocks the Curative Pass benefit. Members who complete their Baseline Visit may qualify for reduced or $0 cost-sharing on covered services for the plan year. Members who have not yet completed it may face standard cost-sharing. We recommend checking your status in the Curative member portal before scheduling.

Both clinicians accept Curative

  • Dr. Rana Kaleemullah, MD — Founder & Board-Certified Psychiatrist. In-person in Southlake and telehealth across Texas.
  • Josh Fantauzzi, MSN, PMHNP-BC — Psychiatric Mental Health Nurse Practitioner. Evening availability by appointment.

Participation is confirmed per clinician and per plan during benefits verification before your appointment.

  • Ongoing medication management (20–50 minute follow-ups)
  • In-person in Southlake, TX
