SLS Psychiatry Now Accepts Curative Insurance in Texas

SLS Psychiatry now participates with eligible Curative health plans for adult psychiatric care, effective June 1, 2026. Both Dr. Rana Kaleemullah, MD, and Josh Fantauzzi, MSN, PMHNP-BC, are active — for psychiatric evaluation and medication management in Southlake or by secure telehealth anywhere in Texas.

What Is Curative?

Curative is a Texas-founded health insurer that offers employer-sponsored health plans. Their signature feature is the Baseline Visit model: members who complete an annual wellness exam may unlock the Curative Pass benefit, which can significantly reduce or eliminate cost-sharing on covered services — including psychiatric care.

Curative offers two main plan types:

  • PPO — access to in-network providers (like SLS Psychiatry) at lower cost-sharing, or out-of-network providers at higher cost-sharing
  • EPO — in-network only, with no out-of-network coverage except emergencies; SLS Psychiatry is in-network, so EPO members have full access

Your specific benefits depend on which plan your employer selected and your Baseline Visit status for the current plan year.

The Baseline Visit Matters Before You Schedule

If you have a Curative plan, check your Baseline Visit status before scheduling psychiatric care. Completing your annual Baseline Visit with your primary care provider is what activates the Curative Pass benefit. Members who have not yet completed it for the current plan year may face standard cost-sharing instead.
