SLS Psychiatry participates with certain Curative health plans for eligible adult members, effective June 1, 2026. Both Dr. Rana Kaleemullah, MD, and Josh Fantauzzi, MSN, PMHNP-BC, are active — for psychiatric evaluation and medication management in Southlake or by secure telehealth anywhere in Texas.
What Is Curative?
Curative is a Texas-founded health insurer that offers employer-sponsored health plans. Its Baseline Visit program helps members maintain plan benefits. Covered services, provider participation, and any cost-sharing depend on the applicable plan terms.
Curative offers two main plan types:
- PPO — access to in-network providers (like SLS Psychiatry) at lower cost-sharing, or out-of-network providers at higher cost-sharing
- EPO — in-network only, with no out-of-network coverage except emergencies; SLS Psychiatry is in-network, so EPO members have full access
Your specific benefits depend on which plan your employer selected and your Baseline Visit status for the current plan year.
Check Your Baseline Visit Status
Curative says members and dependents aged 18 and older must complete the annual Baseline within 120 days of the plan start to maintain its $0 benefits for covered care. The process includes onboarding and a video clinical check-in with a Curative clinician. Do not substitute an appointment with your own primary care provider.

Both Providers Are Active
Dr. Rana Kaleemullah, MD, board-certified psychiatrist and founder of SLS Psychiatry, and Josh Fantauzzi, MSN, PMHNP-BC, psychiatric mental health nurse practitioner, are both credentialed with eligible Curative plans. As always, Participation is verified per clinician and per plan during benefits verification before your appointment.
